A face to face visit or a visit within the last 6 months with provider mentioning nebulizer use in the note, is sufficient to support neb request to Acelleron.

If an adult face mask is needed, and they are getting the nebulizer that day, just write on the order we send, an adult face mask needed, and they will ship it. If Adult face mask needed, no neb dispensed, fax a prescription to Acelleron and the patients demographics, insurance card to 978-824-8766

1. Fill out order form for Accellron
2. Have patient sign, give them yellow copy
3. Have MD sign prescription
4. Fax form, signed prescription, and face to face notes to Accelleron
5. List pt's name and date of when you gave the nebulizer to keep track
6. Mail info to Acelleron on the 15th or 30th of each month
