Fill out accordingly, MD will pick dose and brand of SCIG. Fax application, copies of insurance cards and LOV notes to Accredo 866-233-7151

Follow up with Accredo or co. used. They usually need paperwork to support request: O/V notes, pre and post pnuemo titers/ tetanus titers
