**Wellsense Insurance plan (with Express Scripts PBM):**

-prefers tacrolimus 0.03% or 0.1% over pimecrolimus 1% 

-both tacrolimus and pimecrolimus require PA's, but pimecrolimus PA will only be approved if tacrolimus was tried and failed first

-allowable amount of 30g per 30 days for topicals unless reason of medical necessity for higher quantity is specified
 
-Will only approve the xolair prefilled syringe or auto-injector through pharmacy benefits for @ home administration


**BCBS Federal Employee program:** 

-Will not cover Dupixent if the patient hasn't tried / failed other biologics including xolair and nucala
