 MD must place orders into epic. NO PAPER ORDERS!

 Danvers location is not taking IVIG patients at this time


After orders placed patient should call infusion center for apt / insurance coverage
  * for Medicare they do own PA's
  * if other insurance call "outpatient coordinator" and they will review if needs PA

===== Beverly/Lahey Hospital =====

  * Only has Privigen for IVIG
  * Print out previous order
  * Complete new order and have MD review and sign - Order sheets 3 pages, please have MD sign ALL
  * Fax to Bev Hosp Medical Day Care with LOV notes
